The Process Failure Modes and Effects Analysis (PFMEA) diagram is a tool used to evaluate possible failures and how they may be caused. It details the various problems encountered during the assembly and the "countermeasure" actions required to fix the problems. The diagram consists of the process function, potential failure modes, potential failure effects, potential causes of failure, current process controls, recommended actions, responsible person and target date, and the taken actions. Through these steps, this diagram allows one to see the potential failures in the production process along with how much risk is associated with each. It also organizes these potential prob


The first step, the process function, is an action that must be completed in the assembly process. lems in order to do away with or decrease the probability of these errors occurring in the future. For example, something like the batteries did not work then the radio wouldn't work at all. This means that the problem has a bigger effect on the production of the product as a whole. The RPN equals the product of the severity of the problem, the number of occurrences of each problem, and the likelihood of the problem being detected and corrected before moving onto the next step in the assembly process. Examples of this would be connecting wires to springs, connecting the speakers, connecting the AM or FM tuners, etc. Overall this process is a way to organize possible risks in order to minimize them from a management's perspective. After the steps have been listed along with their potential failures, the third step of current process controls begins. The second step lists the potential defects or errors that could occur. To provide a quantitative analysis of how severe the problem was in the overall process, we calculated the RPN, the risk priority number. At this step, you identify what the possible effects of a defect or error could be. These are specific errors that can be fixed or controlled. This is something that can be fixed and controlled, therefore is a potential cause of a specific failure mode. The higher the RPN, the bigger the risk priority the problem has.
