The relationship between the doctor and the patient is a hateful one because the doctor and the patient grows to hate the girl thanks to her parents, who think by telling their daughter to cooperate or else, make things worse for the doctor. The doctor calls the girl a "savage brat" at one point because she tried to claw his eyes out but missed. The girl was making things so difficult that the doctor was beginning to loose control of his senses and wanted to use force on the girl and enjoy it. The doctor says, "it was a pleasure to attack her" on p. 763. The doctor has had it and needs to diagnose her before is too late. Ironically, by using force is the only way to save her life. On the other hand, the girl is afraid of him and because she thinks the doctor will kill her, she hates him. She uses all her strength to keep him from getting the culture sample. At the end, the doctor uses force on her and is able to dominate her with the help of her own father. The doctor discovers the girl has diphtheria and has had it for at least 3 days.
             The relationship between the doctor and the parents of the patient is one of hatred for the doctor. The mother keeps telling the girl what a "nice man the doctor is" and how "the doctor won't hurt her if she cooperates". The doctor is struggling with those words because the reality is that he isn't a nice man because he has to diagnose her and that will hurt her. The doctor at one point mentions that he would like to "kill" the father because every time he would get close to getting a culture sample the father would let go of the girl and the doctor would have to start all over.
